Transitions began as a way to meet the needs of individuals with a life limiting disease who do not qualify for other healthcare benefits. At Hospice & Palliative Care of Washington County, Transitions™ was funded as a three-year, volunteer-based seed project by the Meyer Memorial Trust. Additionally, five other local community funding bodies, recognizing the unmet needs for the seriously ill, have contributed to this program as well.

When you or someone you love is diagnosed with a terminal illness, it affects your life in many ways. It drains your energy and your spirit, often leaving you unable to manage even the most routine tasks. Transitions is a volunteer based program that provides trained and experienced volunteers to assist individuals facing terminal illness and their families.

Working with referral sources such as care centers, home health agencies, discharge planners and physicians, a Transitions coordinator meets with the client and their family to identify and engage avenues of support according to need. Caring, trained and experienced volunteers offer support, friendship, and respite to clients, often helping with errands, providing transportation to a medical appointment or treatment, support group meetings or simply offering a respite break to family and caregivers.

Transitions is available at no charge to any resident of Washington County who has been diagnosed with a life-limiting illness and who may or may not be undergoing curative treatment. It is available wherever the client resides; in their own home, assisted living center or nursing facility.

To be eligible for
Transitions:

 

 

 

 

A client must be diagnosed with a life-limiting illness and have a prognosis of about one year. Eligibility is not contingent on whether or not the client is receiving curative treatment.

  • Transitions offers support services and our volunteer provide: Friendship Help with errands Transportation to physician offices or treatment
  • Respite break for the family or caregivers
