The following services are offered to each patient and family that are served by Hospice & Palliative Care of Washington County.

Team Approach

Physicians, nurses and social workers, with options as needed for therapists, bath aides, chaplain, and trained volunteers provide medical care and support services not only to the patient but the entire family.

24-Hour Nurse Availability

Never more than a phone call away; we provide 24-hour nurse availability for any questions or emergency.

Volunteer Availability

Volunteers provide respite, companionship, emotional support and other services on a short-term basis.

Medical Social Services

A medical social worker will assist with counseling, financial matters and community resources.

Continuity of Care

Wherever the patient is, hospice care follows; hospital, nursing home, foster care facility or their own residence.

Home Health Aides / Homemaker Services

Aides provide personal care in the capacity of bathing, bed changing, shampooing and other services.

Grief & Bereavement Support

On-going support is provided through the 13-month bereavement program.

To qualify for Hospice a patient should:

  • Live in Washigton County or its outlying areas
  • Have a caregiver or a plan in place when the patient is no longer able to do daily care. Have a physician certify that the patient is beyond the active phase of curative treatment for the disease.
  • It shows the patient has a limited life expectancy of about six months or less and that he or she now desires comfort or palliative measures of care

Who pays for hospice care?

Care is provided by Hospice & Palliative Care of Washington County regardless of ability to pay. Medicare eligible persons obtain care as a fully covered benefit. All medications, hospital stays and equipment needed for the management of the terminal disease is paid for by Medicare-certified hospice organizations.

Many private insurance policies pay for hospice care also. The provision of hospice care for those who do not have the funds is subsidized by contributions from the Washington County Hospice Foundation.
