If you are still seeking curative treatment we may be able to provide palliative assistance and care. Your wishes and needs, including those of your family, are the primary focus of our personalized care plans. LEARN MORE |
ONLINE END-OF-LIFE SURVEYThis survey is about end-of-life issues such as the kind of care you want, the wishes you have, and the choices available to you regarding end-of-life care. Your answers are vital in assisting us to support people in dealing with end-of-life issues and designing support programs for our aging population. Some of the topics may be difficult to consider, and we greatly appreciate your participation in this survey. Your responses can be anonymous, if you choose. However, by providing your name and email address, you will be entered to win one of five (5) $100.00 gift certificates! You can still be assured nothing you share with us will be reported in any way that you could be identified and all information will be kept entirely confidential.
